This role is responsible for managing a caseload of clients receiving aged care or NDIS-funded services.

Key Responsibilities:
  • Develop and implement service agreements to ensure effective support for clients, promoting efficient resource allocation.
  • Address feedback about services and assess service referrals to guarantee funding eligibility, ensuring compliance with relevant standards.
  • Monitor client budgets and costings to maintain financial integrity, preventing unnecessary expenditures.
  • Conduct in-home visits to better understand client needs and preferences, fostering empathetic relationships.
  • Ensure high-quality service delivery by adhering to Aged Care and NDIS standards.
Essential Requirements:
  • Experience or qualifications in Disability Services or Aged Care Services, preferably within a similar role, demonstrating expertise.
  • Excellent interpersonal and communication skills to effectively engage with clients and stakeholders, building strong connections.
  • A genuine desire to make a difference in the lives of clients and their families, driven by empathy and compassion.
  • NDIS Worker Screening Check (or willingness to obtain), ensuring a secure working environment.
  • Current Australian 'C' Class Drivers Licence with no restrictions, facilitating convenient travel.
  • Exceptional time management and organisational skills to prioritise workload and meet deadlines, maintaining productivity.
  • Administrative skills with the ability to multitask in a dynamic environment, adapting to changing circumstances.
