Job Overview

We are seeking a highly skilled Coordinator to manage a caseload of clients in need of support with their aged care or NDIS-funded services. Your primary responsibilities will include connecting clients with internal and external resources, guiding them through the process of achieving their defined goals, and ensuring they receive the best possible care.

Key Responsibilities:
  • Develop and implement service plans that cater to individual client needs.
  • Address client feedback and assess referrals to ensure funding eligibility.
  • Monitor client budgets and costings to optimize resource allocation.
  • Conduct home visits to understand client needs and provide personalized support.
  • Ensure compliance with aged care and NDIS standards.
Essential Requirements:
  • Relevant experience or qualifications in disability services or aged care services.
  • Excellent interpersonal and communication skills.
  • A genuine desire to make a positive impact on people's lives.
  • NDIS Worker Screening Check (or willingness to obtain).
  • Current Australian driver's license with no restrictions.
  • Exceptional time management and organizational skills to prioritize workload and meet deadlines.
  • Administrative skills with the ability to multitask in a dynamic environment.
  • A positive attitude and strong work ethic.
