Key Roles and Responsibilities

We are seeking a skilled Coordinator to support individuals through their life journey. The successful candidate will have experience working with people in the community, strong engagement skills and excellent communication abilities.

Main Tasks:

  • Assist individuals with accessing support services, plan development, implementation of funded supports and reviews.
  • Help individuals build confidence, independence and informal support networks.
  • Ensure timely and high quality service delivery in line with performance targets.
  • Build partnerships with local organisations, attend events and promote inclusion through grassroots engagement.
  • Use systems to document interactions, monitor progress and identify service gaps.

Required Skills and Qualifications

  • Experience supporting individuals in community settings.
  • A strong understanding of participant pathways and planning processes.
  • Ability to manage caseloads and meet service delivery targets.
  • Excellent communication, interpersonal and problem solving skills.
  • Strong community engagement and stakeholder relationship skills.
