Job Title: Community Care Coordinator

Role Overview

This position sits within the SVHM Healthcare for the Homeless program and provides proactive care coordination for clients with complex psycho-social and healthcare needs.

  • Maximise patient functional independence through assessment, planning, implementation and evaluation of care.
  • Conduct holistic assessments in collaboration with clients using motivational interviewing and self-management approaches.
  • Identify and access multidisciplinary and specialist supports to address client issues.
  • Facilitate client-generated goals that are realistic and time-limited; coordinate care with clients, carers and families.
  • Develop collaborative care plans with the health care team; ensure linkages with hospital and community services.
  • Communicate effectively with team members, stakeholders and external service providers; advocate for clients as needed.
  • Manage documentation, risk assessment and escalation; ensure appropriate use of funding and resources.
  • Provide discipline-specific clinical management, discharge planning and community liaison for Cottage patients.
  • Monitor health outcomes and plan transitions from the program with safe long-term community supports in place.
  • Contribute to quality initiatives, service development and team processes; meet Activity Based Funding targets.
Required Skills and Qualifications
  • Bachelor or Master's Degree in Nursing.
  • Registration with AHPRA.
  • Completion of basic life support training/CPR/First Aid.
  • Minimum of 3 years post-registration experience.
  • Current Victorian Driver's Licence.
