Job Title: Care Coordination Specialist

The role of the Care Coordination Specialist is a critical component of our organisation's commitment to delivering exceptional care and services to vulnerable individuals.

This position involves working closely with our Home Care teams to provide support and assistance in scheduling the provision of care and contractor services to our clients.

Key responsibilities include:

  • Coordinating client and staff schedules within enterprise agreement and staff contractual conditions, in consultation with Care Managers and Service Managers;
  • Triage enquiries and respond to unscheduled changes and service delivery requirements;
  • Allocation of fleet vehicles as required;
  • Maintaining positive relationships with clients and employees in a call centre environment;
  • Building relationships and ensuring consistency of processes across our Home Care sites.
