About the Role

We are seeking an experienced Social Worker to join our team in a rewarding career that makes a difference in people's lives. The Social Work Renal program is looking for a Grade 1 Social Worker to work across various sites, including Casey, Clayton, Cranbourne, Dandenong, Moorabbin and the Kingston Centre.

This full-time, permanent role is primarily based at Monash Medical Centre, with work predominantly based at Casey, Dandenong and Cranbourne Dialysis Units, but requiring occasional work across all sites.

The ideal candidate will have experience working with individuals with chronic illness and hospital social work experience. They will be able to showcase their skills in working with people from diverse backgrounds, completing complex psycho-social assessments and developing patient-centred care plans.

This clinical role requires participation in the Emergency Department and on-call rosters.

Key Responsibilities
  • Provide strong focus on patient-centred care
  • Adapt to changing settings and shifts during the business week and weekends or public holidays
  • Develop and implement comprehensive psychosocial assessments and discharge plans
  • Work collaboratively and effectively in a team
Required Skills and Qualifications

The successful candidate will possess:

  • An AASW accredited Bachelor degree or entry-level Master's Degree of Social Work
  • Experience in a healthcare setting, preferably in hospital social work
  • Good organisational skills and a positive attitude towards work
  • Experience in working with culturally and linguistically diverse communities
  • A current Victorian drivers licence
