Dementia Care Specialist

This is a leadership role providing direction for clients with complex dementia care needs within the Western NSW region.
The Aged Care Division offers a range of clinical and non-clinical services across the district to support older people and their families, enabling them to remain supported at home.
The aged care teams are committed to advocacy, restorative care, and reablement to wellness and independence. The role involves working within a strong leadership team and adapting to challenges in a changing healthcare environment.
Key Responsibilities:
- Providing leadership and guidance to care teams
- Developing and implementing care plans that meet the needs of clients
- Working closely with other healthcare professionals to ensure seamless care delivery
