Care Coordinator for Regional Aged Care Facilities

**About Juniper Aged Care**
We are seeking a compassionate and dedicated Registered Nurse to join our team in regional Western Australia.
**Salary & Benefits:** Our competitive salary range is between $101,653 to $126,379 per annum + 11% superannuation. We also offer subsidised accommodation and relocation assistance for those relocating to the region.
**Job Description:** As a Registered Nurse at Juniper Aged Care, you will work collaboratively with healthcare professionals to assess residents' needs, develop and implement care plans, and ensure high standards of care. You will also mentor and support care staff, ensuring compliance with AN-ACC assessments to maintain top-quality care delivery.
**Key Responsibilities:
- Care Planning: Develop and implement tailored care plans for residents, addressing their unique needs and requirements.
- Communication & Collaboration: Work closely with families and carers to address concerns and requests, in collaboration with the Residential Manager.
- Ongoing Training: Stay up-to-date with aged care documentation requirements and provide guidance to care and service staff.
- Holistic Care: Collaborate with healthcare professionals to provide comprehensive care for residents, including wound management, diabetes care, and medication systems.
- Mentorship & Support: Provide ongoing mentoring and support to care and service staff, enhancing their performance and skills.
- Compliance: Ensure adherence to aged care standards and regulations, promoting a culture of excellence within the organization.
**Required Skills & Qualifications:**
- A current registration with the Nursing and Midwifery Board of Australia (AHPRA).
- New Zealand citizenship or permanent residency with unrestricted working rights in Australia.
- Experience in delivering clinical care within aged care or healthcare settings.
- Strong planning, coordination, and mentoring skills.
- Ability to guide staff in wound management, diabetes care, and medication systems.
- Excellent communication, problem-solving, and teamwork skills.
