This is an exciting position as a Nurse Practitioner, embedded in a dynamic and supportive interdisciplinary team that covers the North Shore from Deep Cove to Horseshoe Bay.

As a medical consultant for patients who lack access to short-term/urgent primary care and have been discharged from the Emergency Department or hospital, you will be part of a program focusing on rapid response, reablement, and stabilization for the client.

You will work closely with care providers from Lions Gate Hospital, collaborating with other healthcare professionals, including RNs, Physiotherapists, Occupational Therapist, Pharmacist, and allied healthcare professionals. This role offers flexible full-time employment, allowing you to diversify your practice while being supported by NP Professional Practice and Medical Leadership.

  • Conduct comprehensive assessments of patients and clients, diagnosing diseases, disorders, and conditions.

  • Provide professional guidance to other health professionals and participate autonomously as a member of the interdisciplinary health teams.
  • Collaborate with clients and other health professionals to identify and assess trends and patterns that have implications for clients, families, and communities.
  • Develop and implement population-based strategies to improve health and participate in peer, program, and self-review to evaluate outcomes at the client, community, and population level.
