We are seeking a skilled and experienced CDM Nurse to join our team at ForHealth Medical Centres. As a CDM Nurse, you will play a vital role in supporting GPs and their patients to identify and manage chronic conditions, ultimately keeping them out of hospital.
Key Responsibilities:- Develop and implement care plans to help patients manage their chronic conditions
- Empower patients to take ownership of their health and well-being
- Coach patients on healthy behavior change principles
- Facilitate access to ongoing care from a multi-disciplinary team
- Conduct health assessments to identify chronic disease early
- Collaborate with GPs to promote preventative healthcare
- Review and update patient care plans regularly
- Manage administrative tasks related to CDM appointments
- AHPRA Registered Nurse preferred
- Proven experience in Chronic Disease Management and health assessments
- Knowledge of Health Assessment is highly regarded
- Strong organisational and time management skills
- Outstanding communication and customer service skills
- A passion for people and a personable approach
