In this role, you will play a vital part in delivering high-quality care management and coordination services to our clients.

Your primary focus will be on working collaboratively with healthcare professionals, clients, and their families to develop and implement effective care plans that address their unique needs and goals.

You will be responsible for conducting regular assessments and reviews to ensure the effectiveness of care plans and communicating effectively with clients, families, and healthcare professionals to ensure seamless care delivery.

Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with clients and healthcare professionals
  • Conduct regular assessments and reviews to ensure the effectiveness of care plans
  • Collaborate with multidisciplinary teams to promote holistic care and support
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care delivery
  • Work collaboratively with other healthcare professionals to ensure comprehensive care delivery

Requirements:

  • Degree qualification in nursing or allied health
  • Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA)
  • 3+ years of clinical or health management experience in Nursing or Allied Health
  • Experience within a health or community aged care setting
  • Ongoing commitment to Continued Professional Development and Education
  • Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion
