CLINICAL CARE COORDINATOR (CCC) – RELOCATE TO TIN CAN BAY, QLD

The Role:

As the Clinical Care Coordinator, you'll take charge of reviewing and coordinating care plans, supporting new admissions, and mentoring the clinical team to ensure the highest standards of care and compliance.

Key Responsibilities:

- Lead and oversee clinical assessments and care planning
- Collaborate with RNs and ENs to deliver exceptional care
- Ensure documentation aligns with the Aged Care Quality Standards
- Lead clinical handovers and contribute to clinical meetings
- Drive continuous improvement initiatives
- Assist with incident and infection management, SIRS, and audits
- Support accreditation and regulatory compliance

What We're Looking For:

- AHPRA Registration as a Registered Nurse
- 5+ years of nursing experience in Australia – Aged Care preferred
- Strong leadership and interpersonal skills
- Proven ability to manage clinical governance and risk
- Understanding of SIRS, Restrictive Practices, Incident Management Systems, and Aged Care Quality Standards
