Job Overview

Key Responsibilities:

  • Working with complex safety and discharge planning for people in need
  • Collaborating with healthcare teams to build strong referral pathways to local community services
  • Rotating across different clinical areas, including medical, surgical, rehabilitation, paediatrics, and maternity and special care nursery
  • Mentoring colleagues, supervising students, and contributing to district-wide initiatives
