About the Role

Key Responsibilities
  • Client Care: Provide personalized care and support to clients, including assistance with daily living activities, medication management, and promoting independence.
  • Team Collaboration: Work closely with the care team to develop and implement holistic care plans that meet the unique needs of each client.
  • Communication: Maintain effective communication with clients, families, and other healthcare professionals to ensure seamless care coordination.
  • Administrative Tasks: Complete accurate and timely documentation, including care plans, progress notes, and other administrative tasks as required.
Requirements
  • Qualifications: Certificate III in Individual Support, Aged Care, or Home and Community Care.
  • Experience: Previous experience in a care or support role, preferably in a home care setting.
  • Skills: Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Certifications: Current Police Screening Certificate, NDIS Worker Screening Check (Yellow Card), and Working with Children Check (Blue Card).
