This is an exceptional opportunity to support people living with dementia and their families, caregivers, and clinical staff. You will be part of a centre initiative that aims to provide comprehensive care and support to individuals transitioning from hospital settings back to their homes or into residential care.

Key Responsibilities
  • Provide assessment and biopsychosocial approaches to support individuals with dementia.
  • Implement practical, innovative, and non-pharmacological strategies to prevent, minimize, or respond to changes in behavior related to dementia.
  • Work collaboratively with multidisciplinary teams to develop and implement behavior support plans.
  • Develop reports that are contextualized to individual situations and people, effecting change on the ground and improving quality of life for individuals living with dementia.
  • Undertake assessments to understand the holistic experience of individuals and their care networks.
Requirements
  • Expertise in supporting people living with dementia.
  • Experience in Australian residential aged care and/or community aged care.
  • Excellent negotiation, communication, and relationship-building skills.
  • Ability to prioritize multiple tasks and meet deadlines.
  • Current Australian driver's license and willingness to travel.
  • Empathy and patience.
  • Relevant qualification in Allied Health, Nursing, Aged Care, or Mental Health.
