Care Designer Job Description

We are seeking a dedicated and passionate Care Designer to provide exceptional home care services. As a Care Designer, you will be responsible for delivering high-quality care and support in the comfort of our clients' homes.

Key Responsibilities:

  • Design personalized care plans: Collaborate with clients to create tailored care plans that meet their unique needs and goals.
  • Consult with potential clients: Meet with potential clients to discuss their options for premium home care services.
  • Maintain clinical standards: Oversee client clinical care and maintain accurate documentation records.
  • Lead a team: Assist in the management and development of a growing team of caregivers and nurses.

Requirements:

  • AHPRA Registration
  • Registered Nurse
  • CPR & First Aid Certification
  • Car & Current Victorian Drivers License
  • National Police Check (Aged Care/ Vulnerable Persons Sector)
  • Working With Children Check
  • Disability Workers Exclusion Scheme Approval
  • NDIS Worker Orientation Module Completion
