About the Role
We are seeking a dedicated and compassionate Nurse Care Specialist to join our team at Regis Aged Care Homes. As a key member of our clinical team, you will be responsible for delivering exceptional care to our residents, ensuring their physical, emotional, and social needs are met.
Key Responsibilities
- Lead the coordination and delivery of person-centered care, working closely with our carers and other healthcare professionals to ensure seamless care transitions.
- Conduct thorough assessments of residents' needs and preferences, developing comprehensive care plans that promote independence and choice.
- Manage and safely administer medications, maintaining accurate and up-to-date documentation of resident care.
- Work collaboratively with our carers to deliver our Continuity of Carer model, ensuring residents receive consistent and high-quality care.
- Develop and maintain strong relationships with residents, their families, and our carers, promoting a culture of respect, empathy, and understanding.
Requirements
- A Bachelor of Nursing degree or recognized equivalent qualification.
- Current AHPRA registration (Nursing).
- Experience in complex nursing clinical services and/or aged care settings, hospitals, or community care.
- Experience in geriatric medicine, palliative care, or rehabilitation is highly desirable.
- Ability to work collaboratively as part of a multidisciplinary team, promoting a culture of respect, empathy, and understanding.
