**Physiotherapist Role - Community Aged Care Service**

This is a rewarding position in our multidisciplinary team, where you will work collaboratively to deliver high-quality physiotherapy services and activities to improve the well-being of clients in their own homes and at our Wellness Hubs.

We are seeking an experienced Physiotherapist to join our Community Aged Care team, working on a Full-Time or Part-Time basis. As a key member of our team, you will be responsible for assessing, planning, and delivering individualized/group physiotherapy services and activities to meet the unique needs of our clients.

  • Main Responsibilities:
  • Conduct comprehensive assessments of clients' physical and functional abilities to identify their needs and develop tailored treatment plans.
  • Deliver evidence-based physiotherapy interventions to address mobility issues, pain management, and other age-related conditions.
  • Maintain accurate records of client progress, including exercise sheets, assessment reports, and progress notes.
  • Collaborate with interdisciplinary teams to provide holistic care and support to clients and their families.
  • Educate clients and families on preventive measures and exercises to improve and maintain mobility.

Requirements:

  • Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration.
  • Excellent interpersonal, verbal, and written communication skills.
  • Demonstrated experience working in a multidisciplinary team environment.
  • Effective time management skills and ability to prioritize tasks efficiently.
  • Demonstrated computer literacy and proficiency in relevant software applications.
