Home Care Physiotherapist Opportunity

This role offers a unique chance to work in community aged care, delivering individualized physiotherapy services and activities to clients in their homes and across wellness hubs.

  • We are seeking an experienced physiotherapist to join our multidisciplinary team and provide comprehensive assessments, develop and deliver treatment plans, and implement evidence-based interventions to address mobility issues, pain management, and age-related conditions.
  • The ideal candidate will have expertise in conducting thorough client evaluations, creating tailored treatment plans, and implementing effective physiotherapy strategies to enhance client well-being.

Key responsibilities include:

  • Conducting comprehensive client assessments to understand their physical and functional needs
  • Developing and delivering individualized or group treatment plans based on client goals and preferences
  • Implementing evidence-based physiotherapy interventions to address mobility issues, pain management, and age-related conditions
  • Maintaining accurate records, exercise sheets, assessment reports, and progress notes reflecting client physical function
  • Making recommendations for exercise programs, mobility equipment, and referring to other disciplines as needed
  • Collaborating with interdisciplinary teams to provide holistic care
  • Educating clients and families on preventive measures and exercises to improve and maintain mobility
  • Participating in team meetings, case conferences, and training sessions to contribute to continuous improvement of care delivery
  • Engaging with team members, clients, and their families in a positive and respectful manner, valuing cultural diversity
  • Building relationships with clients and their families, understanding the challenges they face in the ever-changing landscape of aged care

To succeed in this role, you'll need:

  • Bachelor or postgraduate degree in physiotherapy and current registration with AHPRA
  • Excellent interpersonal, verbal, and written communication skills
  • Ability to manage all aspects of compliance
  • Demonstrated experience working within multidisciplinary teams
  • Effective time management skills to ensure timely patient intervention, reports, and outcomes
  • Demonstrated computer literacy
  • Demonstrated knowledge and experience in health, aged, and/or disability services
  • Current driver's license and registered insured motor vehicle
