About This Role

We are seeking a dedicated Registered Nurse to join our team as a Healthcare Service Coordinator. As a key member of our multidisciplinary healthcare team, you will play a vital role in coordinating and supporting the delivery of home-based health care to clients across Australia.

This is an exciting opportunity for a passionate and experienced Registered Nurse to make a real difference in the lives of our clients. You will be responsible for coordinating acute Hospital Care at Home programs for early hospital discharges to home-based care, supporting and communicating with clients remotely, implementing and modifying in-home care plans, and performing administrative tasks as needed.

As a Healthcare Service Coordinator, you will have the opportunity to work closely with multiple stakeholders, including hospital referrers, surgeons/specialists, service providers, and funders. Your excellent communication and interpersonal skills will enable you to build strong relationships and ensure seamless coordination of client care.

Your Key Responsibilities
  • Coordinate acute Hospital Care at Home programs for early hospital discharges to home-based care
  • Support and communicate with clients remotely to achieve their personal and clinical health goals
  • Implement and modify in-home care plans to meet client needs
  • Perform ad-hoc administrative tasks as needed
Your Qualifications
  • Qualification as a Registered Nurse with current AHPRA registration
  • Experience working in a multidisciplinary collaborative team
  • Excellent knowledge of hospital discharge planning
  • Tech savvy – this role requires working across multiple systems
  • Ability to work in a dynamic environment with tight timeframes and competing priorities
