Nurse in Transitional Aged Care

The Transitional Aged Care Program seeks a skilled Registered Nurse to deliver person-centred nursing care in collaboration with individuals and the multidisciplinary healthcare team.
Nurses will supervise and support less experienced clinicians to achieve people-focused quality care, apply evidence-based practice and critical thinking to advocate for and implement individualised person-centred care with a commitment to quality improvement.
They will provide effective clinical communication through information technology, written and verbal means, and strong interpersonal skills.
Required Skills and Qualifications- Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse is mandatory.
- Competencies in and knowledge of providing nursing care within a multidisciplinary team are essential.
- Recent clinical experience relevant to the position, including ongoing professional development and maintenance of competence and skills, is required.

- Occupational Assessment, Screening and Vaccination against Specific Diseases (OASV) Category A is a requirement.
