Care Management Role Overview

Our organization is seeking a highly skilled and compassionate Care Manager to oversee the clinical activities of our team.

This position involves ensuring that our residents' healthcare needs are met through effective communication, collaboration, and leadership.

The successful candidate will be responsible for fostering a culture of excellence in care delivery, adhering to established policies and procedures, and driving continuous improvement in clinical standards.

Key Responsibilities:

About You / Qualifications
  • A current nursing registration with the relevant regulatory body.
  • Proven experience as a Care Manager or senior Registered Nurse within aged care.
  • A commitment to delivering high-quality patient care and a willingness to take ownership of clinical outcomes.
  • Strong leadership and interpersonal skills, with the ability to delegate tasks effectively.
  • An alignment with the organization's values and mission.
  • Demonstrated knowledge of aged care accreditation standards and regulatory requirements.
  • Post-graduate study in relevant disciplines (geriatrics, dementia, palliative care) is an asset.
