**Job Summary:**
We are seeking a highly motivated and empathetic Case Manager to join our team at Challenge Community Services. As a Case Manager, you will play a vital role in supporting children and young people in foster care, working closely with families, carers, government agencies, and regulatory bodies.
Key Responsibilities:
- Manage a variety of cases, performing administrative duties and documenting case notes from face-to-face meetings with children and carers.
- Visit and support children and carers in foster care settings, providing guidance and support as needed.
- Develop and implement individualized plans to meet the unique needs of each child and family.
- Collaborate with internal and external stakeholders to ensure seamless service delivery and achieve desired outcomes.
- Provide education and support to carers, promoting positive outcomes for children and young people.
- Conduct home visits, assessments, and reviews to ensure the well-being and safety of children and young people.
- Develop and maintain relationships with key stakeholders, including government agencies, community organizations, and other service providers.
- Participate in regular team meetings and contribute to the development of policies and procedures.
Requirements:
- Relevant experience in a similar role or any position related to Out of Home Care.
- Empathetic and dedicated, with a commitment to improving the lives of children and young people.
- Confidentiality and professionalism at all times.
- Resilient and adaptable, with the ability to manage competing priorities and deadlines.
- Excellent communication and interpersonal skills, with the ability to build and maintain strong relationships.
- Strong written skills, with proficiency in writing reports and completing formal documentation.
- Computer literacy, with proficiency in using the MS Office suite of products.
- Ability to work well both independently and as part of a team.
- Flexibility, with the ability to work non-standard hours as required.
- Current NSW Working With Children Check (or willingness to obtain prior to commencement), NDIS Workers Screening Check (or willingness to obtain), and full COVID vaccination status.
- Possession of an Australian Driver's License.
