About the Role

**Job Summary:**

We are seeking a highly motivated and empathetic Case Manager to join our team at Challenge Community Services. As a Case Manager, you will play a vital role in supporting children and young people in foster care, working closely with families, carers, government agencies, and regulatory bodies.

Key Responsibilities:

  • Manage a variety of cases, performing administrative duties and documenting case notes from face-to-face meetings with children and carers.
  • Visit and support children and carers in foster care settings, providing guidance and support as needed.
  • Develop and implement individualized plans to meet the unique needs of each child and family.
  • Collaborate with internal and external stakeholders to ensure seamless service delivery and achieve desired outcomes.
  • Provide education and support to carers, promoting positive outcomes for children and young people.
  • Conduct home visits, assessments, and reviews to ensure the well-being and safety of children and young people.
  • Develop and maintain relationships with key stakeholders, including government agencies, community organizations, and other service providers.
  • Participate in regular team meetings and contribute to the development of policies and procedures.

Requirements:

  • Relevant experience in a similar role or any position related to Out of Home Care.
  • Empathetic and dedicated, with a commitment to improving the lives of children and young people.
  • Confidentiality and professionalism at all times.
  • Resilient and adaptable, with the ability to manage competing priorities and deadlines.
  • Excellent communication and interpersonal skills, with the ability to build and maintain strong relationships.
  • Strong written skills, with proficiency in writing reports and completing formal documentation.
  • Computer literacy, with proficiency in using the MS Office suite of products.
  • Ability to work well both independently and as part of a team.
  • Flexibility, with the ability to work non-standard hours as required.
  • Current NSW Working With Children Check (or willingness to obtain prior to commencement), NDIS Workers Screening Check (or willingness to obtain), and full COVID vaccination status.
  • Possession of an Australian Driver's License.
