Make a real difference in the Eyre & Far North Local Health Network Community Nursing Team.
The Clinical Nurse, Respiratory Coordinator provides support and develops effective respiratory services to reduce emergency department presentations and hospital admissions. Key objectives include:
- Reducing ED presentations and hospital admissions for clients with respiratory conditions
- Shortening length of stay for patients with respiratory illness in EFNLHN hospitals
- Facilitating timely transfer of respiratory patients from Metropolitan health services to their community or residence
- Supporting education, self-management, and pulmonary rehabilitation for participants
Responsibilities:
- Develop and implement respiratory services to reduce emergency department presentations and hospital admissions
- Deliver the SA Health Home Oxygen Program, including assessment, care planning, evaluation and review
- Support vulnerable groups through education and self-management initiatives
- Promote pulmonary rehabilitation and timely transfers of respiratory patients
About You:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia
- At least 3 years full-time equivalent post-registration experience
- Effective communication, problem-solving, conflict resolution, and negotiation skills
- Ability to work within a multidisciplinary team and prioritize workloads
- Experience working with Aboriginal consumers and communities to provide culturally appropriate health services
