Overview

Make a real difference and feel like an important part of the Eyre & Far North Local Health Network Community Nursing Team. The Clinical Nurse, Respiratory Coordinator provides a high level of support and works to develop and implement effective respiratory services with aims to reduce ED presentations and hospital admissions, shorten length of stay, enable timely transfers of respiratory patients, support vulnerable groups, and promote education, self-management and pulmonary rehabilitation. Delivery of SA Health Home Oxygen Program, including assessment, care planning, evaluation and review.

Responsibilities
  • Develop and implement respiratory services to reduce emergency department presentations and hospital admissions for clients with respiratory conditions.
  • Reduce length of stay for patients with respiratory illness in EFNLHN hospitals.
  • Facilitate timely transfer of respiratory patients from Metropolitan health services to their community or residence.
  • Support education, self-management, and pulmonary rehabilitation for participants.
  • Deliver the SA Health Home Oxygen Program, including assessment, care planning, evaluation and review.
About You
  • Must be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia, and hold or be eligible to hold a current practicing certificate.
  • At least 3 years full-time equivalent post-registration experience.
  • Effective communication, problem solving, conflict resolution and negotiation skills; able to work within a multidisciplinary team and prioritise workloads; creative, innovative, and flexible in clinical settings.
  • Experience working with Aboriginal consumers and communities to provide culturally appropriate health services, with the ability to engage and influence improvements to Aboriginal health services.
  • Understanding of Aboriginal culture and ability to develop this knowledge within the team and across the health service.
  • Experience in leadership and direction of student nurses, enrolled nurses, and less experienced registered nurses.
