About the Service
The Individual Apartment Living service is a key area of growth for Sylvanvale. This innovative model of support provides drop-in assistance to individuals with disabilities living in their own apartments in residential unit blocks. Our team partners with disability housing providers to ensure tenants receive the necessary Assistive Technology, while Sylvanvale provides tailored support through our service.
About the Role
We are seeking a highly skilled Team Leader to join our Individual Apartment Living team. As a Team Leader, you will be responsible for mentoring a team of Disability Support Workers to deliver high-quality, client-centered support. Your key responsibilities will include:
- Maintaining knowledge of NDIS Practice Standards and NDIS Operational Guidelines.
- Ensuring high-quality support is provided by Disability Support Workers.
- Facilitating support and supervision of frontline staff and monthly Team Meetings.
- Being the key point of contact for clients and frontline staff.
- Ensuring all client documentation is current and staff are trained in implementing support specific to client needs.
- Ensuring frontline rostered hours do not exceed those consented to in client Service Agreements.
About You
We are looking for a dedicated and innovative people leader who wants to support their team to provide exceptional services to individuals with disabilities. You will be a natural problem-solver, always striving for continuous improvement and delivering high-quality services. Your excellent communication skills will enable you to build strong stakeholder relationships.
Essential:
• Customer-driven approach to service delivery.
• Ability to achieve results and deliver on quality.
• Ability to identify trends and drive process improvement.
• Solid understanding of the NDIS Quality and Safeguarding Framework and Practice Standards.
• Knowledge of providing services and support to people living with disability.
• Working knowledge of budget management.
• Solid report writing skills and data analysis.
• Strong stakeholder engagement skills.
Desirable:
• Certificate IV in Disability or similar.
Successful applicants will require a current Australian Drivers Licence and First Aid certificate and a full COVID-19 vaccination and booster. An offer will be subject to a police clearance, a pre-employment medical, and a NDIS Worker Screening Check.
