Job Overview

We are seeking a skilled Case Manager to join our team in Newcastle. As a key member of our Family Based Care team, you will work closely with children, young people, their carers and families to create and implement case plans that support the permanency goals of those we support.

This dynamic role requires strong relationship building skills, excellent communication abilities and a passion for supporting vulnerable individuals. You will be responsible for collaborating with carers, guiding them in their foster care journey and helping them implement effective strategies that best support the children and young people within their care.

Key Responsibilities
  • Collaborate with children, young people, carers and families to develop and implement case plans that align with the permanency goals of those we support
  • Build and maintain strong, functional relationships with carers and other stakeholders to ensure the needs of the children and young people are met
  • Guide carers in their foster care journey and provide them with the necessary tools and resources to support the children and young people in their care
  • Monitor placements and identify areas for improvement to ensure the best possible outcomes for the children and young people
Requirements
  • Relevant bachelor's degree such as Social Work, Social Welfare, Psychology or similar
  • Previous experience in case management within an Out Of Home Care (OOHC) /Child Protection setting
  • Experience working with children and young people who have experienced trauma, abuse and/or neglect
  • Working understanding of the NSW Permanency Support Program
  • Understanding of trauma and its effects on children and young people
  • Current driver's licence
