We are seeking an experienced Care Coordinator to join our team at Programmed, a leading provider of disability services across Australia. As a Care Coordinator, you will play a vital role in supporting our clients with disabilities, veterans, and aged care programs.

Key Responsibilities
  • Client Management: Provide end-to-end client management and onboarding, ensuring a seamless experience for our clients.
  • Compliance and Quality: Ensure all business activities comply with relevant legislation, codes of practice, guidelines, and ethical standards.
  • Care Team Collaboration: Work with the Care Team to ensure all rosters are adequately filled and provide high-quality care to our clients and support workers.
  • Feedback and Concerns: Act on and address all client and support worker feedback and concerns to ensure retention levels.
  • Business Development: Actively source new referral partners and work on ways to grow our client base.
Requirements
  • Experience: Minimum 5 years' experience working in the disability sector, particularly in the home-care market.
  • Qualifications: Mandatory Health or Community Care qualification or commensurate experience.
  • Skills: Exemplary client relationship management and/or sales skills, highly developed verbal and written communication skills, strong organisational, problem-solving, and time-management skills, outstanding attention to detail, and commitment to the Programmed value of care and empathy.
  • License: Current driver's license.
