Job Role Summary

We are seeking a health and aged care professional to work in partnership with clients and their families, providing support and care within their homes.

About the Job

This is a rewarding opportunity to join our team as a Care Partner, Registered Nurse. In this role, you will be an integral point of contact for workers and clients, building strong relationships and providing expert guidance and advice.

Key Responsibilities:

  • Conduct comprehensive client assessments and develop personalized care plans to support clients in remaining at home.
  • Provide professional health and clinical care advice and direction to a team of home care workers.
  • Ensure all practices are consistent with relevant policies and legislative frameworks.
  • Maintain accurate and up-to-date client records and information.

Qualifications and Experience

To be successful in this role, you must hold:

  • A formal tertiary qualification such as a Bachelor of Nursing or equivalent.
  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • A national criminal history/police check.
  • A valid First Aid and CPR certificate.
  • A valid Working With Children's Check Card.

Desirable Qualities and Skills:

  • Knowledge of the aged care industry/issues or Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles and the Aged Care Quality Standards.
  • Previous experience in Aged Care or related disciplines.
  • Easily adaptable to new software platforms.
