About the Role

The Care Choices Partner plays a pivotal role in supporting older people and their families to navigate the care system with clarity, confidence, and compassion. This exciting opportunity is designed for individuals who possess exceptional customer service skills, outstanding communication abilities, and a strong understanding of the aged care sector.

Key Responsibilities
  • Build relationships with older people and their families to understand their care needs, preferences, and goals.
  • Provide tailored advice on aged care options, including residential care, home care, respite, and NDIS supports, ensuring clients receive the best possible outcomes.
  • Support clients through the entire placement journey, from initial enquiry to transition, including attending tours and submitting applications or documentation on their behalf.
  • Onboard new providers by gathering service details, negotiating partnership terms, and ensuring accurate system setup.
  • Maintain up-to-date and accurate records of provider offerings, pricing, availability, and client interactions.
  • Collaborate with key stakeholders to improve service quality and client experience.
Requirements
  • Experience in aged care, retirement living, disability support, or healthcare navigation.
  • Proven experience in a client-facing role within a high-trust environment.
  • Strong customer service ethos with a commitment to delivering exceptional, personalised experiences.
  • Outstanding communication skills – clear, compassionate, and confident across phone, email, and in-person interactions.
  • Highly organised with the ability to manage multiple tasks, timelines, and client needs simultaneously.
  • A problem-solver with sound judgment and a proactive, solutions-focused mindset.
  • Strong attention to detail and ability to maintain accurate records and documentation.
  • Tech-confident: comfortable using CRMs, cloud-based systems, and able to adapt to new technologies with ease.
  • Works well independently and collaboratively as part of a close-knit team.
  • Knowledge of the Aged Care Act, CHSP, Home Care Packages, NDIS, or other relevant legislation.
  • Qualifications in aged care, nursing, allied health, social work, community services, or similar.
  • Understanding of the residential aged care and/or retirement living sector.
  • Familiarity with aged care assessment processes and placement pathways.
