
Empowerment Support Specialist

About the Opportunity:
We deliver high-quality support services to individuals with disabilities and their families. Our goal is to empower participants to navigate the NDIS system, achieve their goals, and link into funded and community supports.
As a Support Coordinator, you will work with participants to identify and develop strategies to enhance their capacity for self-advocacy, independence, and social inclusion.
You will provide individualized support to participants by:
- Navigating the NDIS system and identifying potential barriers to service delivery;
- Developing and implementing personalized support plans that meet the unique needs of each participant;
- Liaising with internal and external stakeholders to ensure seamless service delivery;
- Providing education and training to participants on their rights, responsibilities, and entitlements under the NDIS.
About You:
We are seeking an experienced and passionate professional who possesses:
- The ability to work autonomously and collaboratively as part of a team;
- Strong communication and interpersonal skills, with the ability to build rapport with participants, families, and stakeholders;
- A proven track record of working effectively in diverse settings and cultures;
- Knowledge of the NDIS and its operations;
- Experience in providing support coordination services to individuals with complex needs.
