Primary Health Care Nurse

We are seeking a dedicated and experienced Primary Health Care Nurse to join our team in the Limestone Coast region.

As a Primary Health Care Nurse, you will be responsible for providing high-quality primary health care to clients living in the community.

Key Responsibilities:

  • Assess and plan patient care to ensure the highest standards
  • Collaborate with other healthcare professionals and community organisations
  • Implement and coordinate services tailored to each patient's needs
  • Use problem-solving, negotiation, and communication skills to drive positive health outcomes
  • Work with diverse groups, including Aboriginal communities, in a culturally sensitive manner

Requirements:

  • Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
  • Ideal experience in community health
  • Ability to work within a multidisciplinary team
  • Ability to engage with patients in a culturally appropriate way
