About the Role

We are seeking a Support Coordinator to join our NDIS team based in Ipswich.

Key Responsibilities
  • Provide client support in accordance with our Model of Practice, including skills development, capacity building, and community connections.
  • Undertake client assessments and reviews, including risk assessments, in accordance with our Model of Practice.
  • Support clients to identify goals and support needs in line with NDIS Support Plans or other funding agreements.
  • Maintain oversight of individual support Service Agreements to ensure contractual obligations are met.
Requirements
  • Tertiary Qualifications in social work, psychology, behavioural sciences, or a human services-related field (or lesser qualifications and demonstrable experience).
  • A strong sense of professional boundaries and integrity.
  • NDIS Workers Screening clearance (or ability to acquire).
  • Knowledge and experience in the NDIS.
  • Excellent organisational and time management skills.
  • Demonstrated skills using client information management systems.
  • Ability to build rapport quickly and effectively with multiple stakeholders.
  • Current driver's licence and access to a registered roadworthy and insured vehicle (minimum third-party property).
