Case Manager

Centacare is seeking a dedicated and compassionate Case Manager to join our team in Whyalla Stuart. As a Case Manager, you will provide specialised case management and crisis support for individuals experiencing homelessness and domestic violence.
Key Responsibilities- Conduct risk assessments and develop safety plans to ensure client safety in every interaction.
- Provide Domestic and Family Violence (DFV) education and supportive counselling, helping clients build resilience and acknowledge their strengths.
- Advocate for clients with internal and external providers, including referral to mental health, family therapy, and other specialised services.
- Coordinate housing support by working with housing providers to secure long-term accommodation.
- Maintain accurate client records and participate in quality improvement programs to meet contractual and legislative standards.
We are looking for a professional with a background in domestic violence, homelessness, or related fields. Your approach should be trauma-informed, client-centred, and culturally responsive. Requirements- Degree in Social Work or relevant experience
- Experience in domestic and Aboriginal family violence, mental health, homelessness, or trauma-informed services (Desirable)
