Join us at CareChoice, a leading NDIS and TAC/WorkSafe registered disability provider, as an Occupational Therapist. We're seeking an experienced professional to join our team in delivering person-centred care services across various settings.

About the Role

We're looking for a highly motivated and skilled Occupational Therapist to fill this exciting opportunity. As part of our multidisciplinary team, you'll be responsible for conducting OT-specific assessments, interventions, and planning within the NDIS and Aged Care frameworks.

Your Key Responsibilities

  • Conduct comprehensive assessments to identify clients' needs and develop individualised plans
  • Deliver evidence-based therapy sessions to improve clients' function and independence
  • Precscribe Assistive Technology and Home Modifications, preparing funding applications as needed
  • Provide ADL and functional capacity assessments, offering recommendations and intervention strategies
  • Manage a caseload of complex clients, ensuring timely and effective support
  • Liaise with referrers, funding bodies, and support workers to ensure seamless service delivery

About You

To succeed in this role, you'll require:

  • A tertiary qualification in Occupational Therapy and current AHPRA registration
  • Demonstrated experience in person-centred care, with a strong understanding of anatomical and neurological principles
  • Knowledge of assistive technology and home modifications, with excellent investigative skills
  • The ability to work effectively in a multidisciplinary team and communicate with clients, families, and stakeholders
