Job Summary
This is an exciting opportunity for a dedicated and passionate Occupational Therapist to join CareChoice as a key member of our team. We are seeking an experienced therapist who can provide high-quality services to our clients, conduct assessments, develop intervention plans, and ensure the best possible outcomes.
About the Role
The successful candidate will have a strong background in occupational therapy, with experience working with clients across various settings, including NDIS and Aged Care. They will be responsible for conducting thorough assessments, developing tailored intervention plans, and delivering evidence-based treatments to improve clients' functional abilities and independence.
Key Responsibilities
- Conduct comprehensive OT assessments, interventions, and planning within the NDIS and Aged Care frameworks to enhance clients' function and independence.
- Develop and implement individualized treatment plans that align with clients' goals and needs.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and achieve optimal client outcomes.
- Prescribe Assistive Technology and Home Modifications funded under the NDIS and other external funding agencies, including preparing funding applications.
- Provide ADL (activity of daily living) and functional capacity assessments within clients' homes, offering recommendations and interventions as needed.
- Manage complex caseloads, prioritizing tasks, and ensuring timely delivery of services while maintaining high-quality standards.
- Deliver information and skills training to clients, their families, and support workers as required to support NDIS goals.
- Liaise with NDIS, Support Coordination, external organizations, and other referrers/funding bodies to ensure effective communication and coordination.
Requirements and Qualifications
- Tertiary qualification in Occupational Therapy
- Current AHPRA Registration
- Current First Aid & CPR Certificate
- NDIS Worker Screening Clearance
- Working with Children Check (Employee type)
- Valid Victorian Driver's Licence with access to a comprehensively insured vehicle
Salary and Benefits
