The Clinical Care Coordinator and Nurse Mentor is a pivotal position within our Edge Hill facility, reporting to the Facility Manager with support from the Care Manager. This full-time role entails supervising and supporting our passionate care team to ensure residents receive exceptional clinical care.
The key objective of this role is to manage and resource care needs within the facility, maintaining up-to-date documentation and assessments aligned with best-in-class clinical practices.
Key Responsibilities- Provision of clinical expertise and leadership to clinical and care service teams across the facility.
- Collaboration with the Care Manager to lead carer teams in meeting all clinical needs of residents, monitoring, and tracking clinical indicators for optimal outcomes.
- Management of assessment, documentation, and submission of AN-ACC requirements within the facility.
- Supporting effective documentation and user support of policies, procedures, and IT system requirements.
- Coordination of assessment, care planning, and review processes within the facility.
- Assistance with the admission of new residents for optimal care assessments and initial consultations with residents and families.
- Maintenance of accurate information recording in accordance with organisational and legislative requirements.
- Input and oversight into facility team rosters, ensuring an optimal skill mix for care delivery.
- Management and documentation of clinical risk within the facility, promoting continuous improvements.
