Senior Psycho-Social Support Specialist

We are seeking a passionate and experienced professional to provide client-centred, strengths-based support to individuals with complex psycho-social needs.
This role plays a vital part in assessment, referral, and treatment planning while mentoring other case managers and contributing to continuous improvement initiatives.
Key responsibilities include:
- Delivering advanced case management to clients with complex needs.
- Developing person-centred support plans through engagement.
- Identifying risks and implementing harm minimisation strategies.
- Preparing and presenting court reports and providing expert advice to the judiciary.
- Mentoring and coaching case managers to enhance service delivery.
- Building and maintaining relationships with legal professionals and service providers.
- Promoting access to services by navigating local referral pathways.
- Contributing to practice improvements and professional development.
