Job Opportunity

We are seeking a proactive, compassionate and forward-thinking professional to join our team as a Care Coordinator. The role involves coordinating care services under the Support at Home framework, ensuring clients are empowered to make informed choices and maintain independence.

This position offers the opportunity to step beyond traditional case management and into leadership in action. A company vehicle is provided for business travel between locations.

About the Role

As a Care Coordinator you will be responsible for delivering person-centred case management tailored to each client's goals and needs. This includes preparing and reviewing budgets with clients, managing a caseload and building strong relationships with clients and families.

  • Deliver individualised care plans that cater to each client's unique needs and preferences
  • Prepare and review budgets with clients to ensure alignment with their care objectives
  • Manage a diverse caseload, conducting regular assessments and reviews to ensure optimal outcomes
  • Foster trusting relationships with clients and families through effective communication and advocacy
  • Coordinate services across allied health, nursing and home support to deliver comprehensive care solutions
Requirements

To succeed in this role, you will require:

  • A natural ability to coordinate and manage multiple priorities
  • Strong problem-solving and advocacy skills to navigate complex care situations
  • Excellent planning, communication and interpersonal skills to build strong relationships with clients and families
  • A valid Australian driver's licence and clean driving record
  • Eligibility to obtain a National Police Check and NDIS Worker Screening Check
