Job Opportunity for Clinical Registered/Enrolled Nurse

We are currently seeking a skilled and compassionate Clinical Registered/Enrolled Nurse to join our community-based care team in Hervey Bay.

Key Responsibilities:
  • Conduct comprehensive assessments of participants in their homes, identifying their unique needs and developing personalized care plans.
  • Monitor and communicate with participants, reporting on their progress and any changes to their condition.
  • Undertake ongoing case management, supporting participants' clinical and holistic needs, and ensuring timely referrals to allied health services when necessary.
  • Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
  • Document participant goals, progress, and outcomes using person-centered strategies, focusing on reablement and enablement.
  • Perform administrative duties, including data entry, processing invoices, and maintaining accurate records of interactions.
Requirements:
  • Advanced computer skills, particularly in CRM software.
  • Excellent administrative and organizational skills, with the ability to prioritize tasks and manage competing demands.
  • Strong problem-solving and multitasking abilities, with a focus on delivering high-quality care in a fast-paced environment.
  • Ability to work effectively in a wellness model, prioritizing reablement and enablement strategies.
