We are seeking an experienced Lifestyle Facilitator to join our team. This rewarding role involves developing and implementing lifestyle programs tailored to the diverse needs of our residents.

About the Role

The successful candidate will be responsible for designing and delivering activities that promote personal choice, autonomy, and social interaction. You will work closely with residents, colleagues, and volunteers to create a supportive and engaging environment.

Key Responsibilities
  • Develop and implement lifestyle programs in collaboration with stakeholders.
  • Conduct assessments to identify individual needs and preferences.
  • Design and deliver activities that cater to diverse interests and abilities.
  • Build relationships with residents, colleagues, and volunteers.
  • Maintain accurate records and reports as required.
Requirements
  • Certificate IV in Community Services (Leisure and Health) or equivalent qualification.
  • Two years' experience in a lifestyle or related role.
  • Excellent communication, interpersonal, and organisational skills.
  • Ability to work in a person-centred model and promote personal choice.
  • Proven ability to assess each resident's social and emotional needs.
  • Understanding of the ageing process and caring for older people.
  • Self-motivation with well-developed organisational skills.
  • Ability to work with a team and excellent customer service skills.
