Healthcare Professional - Community Registered Nurse

Job Overview:

The purpose of this role is to provide quality nursing care and support services to clients in their homes. The successful candidate will be responsible for conducting in-home clinical assessments, providing advice and guidance to clients, and collaborating with local health professionals to ensure seamless care.

Key Responsibilities:

  • Conduct in-home clinical assessments and develop individualized care plans for clients.
  • Provide expert clinical advice and support to clients and their families, as well as referrals to relevant services.
  • Lead and supervise a team of healthcare professionals, including Care Advisors and Home Care staff, to ensure high-quality care delivery.
  • Collaborate closely with local allied health professionals to facilitate responsive and effective care solutions.
  • Develop and implement health education programs, including social support groups and carer education initiatives.

Requirements:

To be successful in this role, you will require a Bachelor of Nursing or equivalent qualification, current AHPRA Div 1 Nursing registration, and a minimum of 3-5 years' experience as a Registered Nurse. You must also possess excellent communication and interpersonal skills, as well as the ability to work effectively in a dynamic team environment.
