Job Title: Case Manager

We are seeking a skilled and compassionate Case Manager to join our team at Alzheimer's Queensland.

About the Role

This is a full-time position that requires the successful candidate to work autonomously and as part of a team to provide high-quality care and support to our clients in the Gold Coast region.

Key Responsibilities
  • Develop and maintain effective care plans that meet the unique needs of each client
  • Provide holistic case management services, including initial intake assessments and ongoing support
  • Build strong relationships with clients, their families, and carers to ensure effective support and care
  • Manage packages, waitlists, and budgets effectively to ensure the best possible outcomes for clients
  • Ensure all care and case coordination is performed in accordance with best practice standards, legislation, and Alzheimer's Queensland's philosophy, policies, and procedures
Requirements
  • Demonstrated experience in aged care, preferably in a Case Manager or Care Coordinator role
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
  • Relevant qualifications, such as a Diploma in Aged Care or a related field
  • Ability to manage a caseload effectively and prioritize tasks
  • Knowledge of Home Care Packages and relevant legislation
  • National Police Clearance Certificate and current First Aid and CPR Certificate
  • COVID vaccinations and a valid driver's license
