We are seeking a skilled and compassionate Case Manager to join our team at Alzheimer's Queensland.
About the RoleThe successful candidate will be responsible for providing high-quality care and support to our clients in the Gold Coast Region. This will involve conducting initial intake assessments, developing and maintaining care plans, and performing holistic case management.
As a Case Manager, you will be required to work autonomously and prioritize effectively to ensure the best possible outcomes for our clients. You will also be responsible for building strong relationships with clients and their families, and for ensuring that care and case coordination is performed in accordance with best practice standards and legislation.
Key Responsibilities- Conduct initial intake assessments and develop care plans that meet the unique needs of each client
- Perform holistic case management to ensure the best possible outcomes for clients
- Build strong relationships with clients and their families to ensure effective support and care
- Ensure that care and case coordination is performed in accordance with best practice standards and legislation
- Manage client packages, waitlists, and budgets effectively
- Demonstrated experience in aged care, preferably in a Case Manager or Care Coordinator role
- Ability to work autonomously and prioritize effectively
- Excellent communication and interpersonal skills
- Relevant qualifications, such as a Diploma in Aged Care
- Ability to manage a caseload and prioritize tasks effectively
- Knowledge of Home Care Packages and relevant legislation
