Overview of Role

Key Responsibilities
  • Offer initial response and assessment via phone, including needs, risks, and vulnerabilities assessment
  • Provide on-site support for walk-ins and deliver appropriate assessment and referral services
  • Establish relationships with clients to promote self-determination and participation
  • Collaborate with clients to develop personalized case plans that address their specific needs or goals
  • Assist clients in accessing relevant support services and establishing community connections
Requirements
  • Relevant tertiary qualification in a field such as Community Services, Social Work, or a related discipline
  • Experience in client assessment and case management within a social service context
  • Previous experience in delivering case management type support over the phone is highly desirable
  • A valid QLD Working with Children Check and Driver's Licence are mandatory
