Senior Clinical Leadership Role

The ideal candidate will be a seasoned clinician with a strong background in care management and clinical leadership.

  • This senior role is focused on delivering high-quality care to clients on the End-of-Life Pathway, ensuring timely, compassionate, and person-centred support.
  • A key aspect of this position is providing clinical oversight for the Restorative Care Pathway under the Support at Home Program, working closely with multidisciplinary teams to develop and implement effective care plans.
  • The successful candidate will be responsible for supervising and supporting Restorative Care Partners, providing clinical guidance and performance oversight to ensure exceptional patient outcomes.
  • Maintaining direct oversight of clinical quality, escalation processes, and model fidelity will also be a critical component of this role.
  • In addition, the Senior Clinical Lead will contribute to iterative improvements based on practice insights, quality data, and stakeholder feedback.
  • This role requires strong collaboration and communication skills, as the Senior Clinical Lead will work closely with internal and external teams to ensure coordinated delivery across care and quality functions.

Key Responsibilities:
