About the Role:
We are seeking a Care Consultant to join our team in the Barwon South Western region. As a Care Consultant, you will work with clients to conduct face-to-face home support assessments and enable access to care for older Australians to remain living independently in their chosen community.
You will use a standardized national assessment process that focuses on wellness, working closely with each client to develop individualized support plans with goals tailored to their needs.

Responsibilities:

- Strong communication and interpersonal skills
