Home Health Care Nurse

This is a dynamic opportunity for a Registered Nurse who is passionate about making a positive impact on people's lives. As a key member of our healthcare team, you will have the chance to work collaboratively with physicians and other healthcare professionals to develop and implement individualized plans of care.
Responsibilities- Patient Assessment: Conduct initial and comprehensive nursing evaluations to determine primary care focus and develop plans of care within state guidelines.
- Care Planning: Develop, implement, and revise plans of care with physicians and healthcare professionals to ensure accurate documentation and effective patient care.
- Patient Education: Educate patients and their families on disease management, treatment options, and self-care techniques to promote optimal health outcomes.
- Collaboration: Work collaboratively with physicians, therapists, and other healthcare professionals to coordinate patient care and achieve desired outcomes.
- Education: Bachelor's degree in Nursing (BSN) or higher.
- Experience: Minimum one year of experience as a Registered Nurse in a home health setting.
- Licensure: Current licensure as a Registered Nurse in the state where the position is located.
- Certifications: BLS certification required; advanced certifications preferred.
