Key Responsibilities- Conduct comprehensive psychosocial assessments and develop individualized care plans
- Provide counseling and support to patients and families, addressing complex social and emotional needs
- Collaborate with healthcare teams to ensure seamless care transitions and coordination
- Develop and maintain relationships with community resources and services to support patient needs
- Participate in quality improvement initiatives and contribute to a positive team culture
- Bachelor of Social Work or Master of Social Work Qualifying OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
- Eligible for ordinary membership of AASW if overseas trained
- A current Victorian Driver's Licence
