Empower Lives through Holistic Support

This role is an opportunity to make a meaningful difference in the lives of individuals and families. As a Housing and Wellbeing Case Manager, you will work in partnership with clients to empower them to overcome identified housing and wellbeing needs.

Key Responsibilities:

  • Provide high-quality supports and interventions based on the program model of care.
  • Engage clients in crisis accommodation, street outreach, and refuges, and translate initial engagement into strong ongoing partnerships.
  • Facilitate client-led assessments and Care Plans to identify client goals, including entire family where appropriate.
  • Provide role-specific targeted interventions and family group meetings to achieve client goals.
  • Collaborate with multidisciplinary teams to provide holistic care and support services.
  • Develop and maintain effective relationships with clients, families, and community stakeholders.
  • Contribute to a culture of excellence, innovation, and continuous improvement within the organization.

Requirements:

  • Cert IV in Social Work, Human Services, or related field, with at least 12 months experience in case management.
  • OR current graduate university bachelor's degree in social work, human services, or related field, with at least 10 weeks full-time placement in social service field.
  • Demonstrated ability to build client agency, self-efficacy, and facilitate change through client empowerment.
  • Ability to work in a performance-driven environment to meet individual weekly, monthly, and quarterly performance targets.
  • Knowledge and understanding of factors contributing to vulnerability, disengagement, and intergenerational trauma of client group.
  • Experience working with high-risk and/or crisis clients, with high-level planning and organizational skills.
  • Current drivers' license and Working with Children's Card (Blue Card) or eligibility to apply.
